Symphony Village Forms and Agreements

Activity Request Form for Clubhouse Use (Online Form) - Residents who would like to use the Clubhouse for an event must complete and submit this form to obtain approval and have the event put on the master calendar. If the resident needs the Clubhouse set up, the particulars must be provided.

Artisans Guild Artist Release and Waiver of Liability - Each resident who wishes to participate in an Artisan’s Guild Art Show must sign this agreement to participate in current or future art shows.
Artisans Guild Show Submission Form - Each resident who participates in an Artisan’s Guild Art Show must fill out this form and attach the bottom part of the form to the back of each piece of artwork submitted.
Calendar Editor Contact Form - Use this form to contact the Calendar Editor to add, change, or cancel a meeting or event on the SV Master Calendar.
Concert Hall Lease Agreement - This agreement must be signed and lease fees paid by the resident hosting an event whenever the Concert Hall is to be used for an event that includes nonresident guests.
Direct Debit Payment Form for Monthly HOA Payments - If a resident wishes to have the monthly HOA fee deducted from their bank account, this form must be filled out and submitted to ProCom.
Directory Update Form - This form is used by new residents wishing to have their address, phone number(s), and email addresses added to the online Resident Directory. This form can also be used to update a resident’s information that is already in the directory. The Directory is password protected for SV Residents.
eBlast Distribution List Subscription Form - The HOA issues official emails (known as “eBlasts”) to Residents who sign up to receive them. This form is used to add your Resident email address to receive future eBlasts.
Emergency Information Form, SV (CERT) - The form contains information about who the occupants of house are, who has key access to the house, if anyone has any disabilities, and other information that might be useful to first responders in an emergency. Completed forms should be returned to the SV Management Office. This form is CONFIDENTIAL and VOLUNTARY.
Exterior Modification Process and Form (Online Form) - This form is required for all exterior modifications including landscaping, roof replacement, lighting, doors, colors, and many other modifications (see form for the full list). Once form is completed, return it to the SV Management Office. The Covenants Committee is responsible for reviewing and approving the exterior modification before any work is started.

Feedback and Suggestion Forms - Click on the appropriate committee below to provide suggestions and comments to the committee. Fill out the online form and click the Submit button.
File of Life Form - The File of Life is a summary of your medical data, emergency contacts, and medications that Queen Anne's County Emergency Personnel (paramedics, EMTs, firefighters) are trained to look for hanging from your refrigerator. Fill out this form, attach it to your refrigerator, and update it frequently.
Ground Installation Form - This form is an addendum to the “Exterior Modification Process and Form” that must be submitted whenever the modification requires digging. To complete this form with a Miss Utility ticket number, contact Miss Utility and they will arrange for verification of public underground lines/pipes and provide a ticket number. An Irrigation Verification must also be obtained and signed off by the Irrigation Company. Full instructions are on the form regarding Miss Utility and the Irrigation Company.
Landscape Service Opt-Out Form - Owners may opt out of front bed maintenance (mulching, weed control, fertilizing, and trimming of shrubs). The opt-out form must be filled out each year and is available in November of the current year and must be submitted before January 4 of the following year. The opt-out will be enforced for the entire year. This form will be available in November 2021.
Lease Addendum Form - This addendum is for any lease agreement where an owner has rented their house to a tenant. Completed addendums should be submitted to Village Management.
Libretto Neighbor of Note Nomination Form - Used to nominate an interesting person for a Libretto article. A Libretto editor will contact you and help you to write an article.
Lifestyle Debit Card Request Form - Request to use the Debit Card to pay for Lifestyle events.
Name Tag Request Form - To order a Symphony Village name tag for any resident, fill out form and send it to Dianne Heffernan at desh14desh@gmail.com.
Pavilion Reservation Form - Residents use this form to reserve the Community Pavilion for an event. Completed forms should be submitted to the SV Management Office for review, approval, and scheduling.
Prayer List Distribution List Subscription Form - This form is used to add your resident email address to our Neighbor-to-Neighbor “Prayer” list. This mailing list is used by Neighbor-to-Neighbor to support residents in their times of need.
Solar System Installation Rules and Guidelines & Application Form - Residents must complete this form if they would like to install a Solar System. Once completed, return it to the SV Management Office. The Covenants Committee is responsible for reviewing and approving the exterior modification before any work begins.
Suggestion Box Form - This form is used to offer ideas, suggestions and comments on how to make our wonderful Community even better. The suggestions will be shared with the Board Members and possibly SV Management where appropriate. General answers will appear periodically in the Libretto.

Work Order Request Forms - From time to time, certain issues may occur with respect to a resident’s landscape, irrigation, trees, or other issues. In order to assess and fix the issue (if necessary), residents must submit a Work Order Request, which will be sent to SV Management for resolution. Click on the appropriate form below. Fill out the online form and click the Submit button.
